<div><strong>INTENSIVE SERVICE COORDINATION CASE MANAGER<br>Full-time Position in New Liskeard</strong><br><br>As a member of the Child and Youth Mental Health team, the Intensive Service Coordination Case Manager works in collaboration with other service providers to provide comprehensive intensive service coordination functions for identified children/youth with multiple/complex special needs for all voluntary services funded by the Ministry of Children, Community and Social Services and the Ministry of Health and Long-Term Care in the rural, remote and urban communities. The successful candidate will develop, co-ordinate and manage the implementation of an integrated service plan to meet the identified needs of the child/youth and family.<br> <br><strong>JOB REQUIREMENTS (QUALIFICATIONS AND SKILLS):</strong></div><ul><li>Graduate or Baccalaureate degree in Human Services.
